Is it true that the risk of having an unhealthy child is greatly increased in a first cousin marriage?

Q. Hello Dr. Daniers, I want to know about first cousin marriage. I’m planning to get married to my first cousin and I have heard that the risk of having a defective baby from a first cousin marriage is high. My first cousin is from my father’s side, i.e. I’m gonna marry the son of my father’s brother. My parents believe that marriage of first cousins is acceptable if its from mother’s side and its less risky when compared to marrying a first cousin from father’s side. Is it true? I would like to know what are the treatments available to have a healthy baby from a first cousin marriage and is it possible for us to find out weaknesses in our DNA and predict if we gonna have a healthy child or not? Please be kind enough to send me a reply at your earliest.

A. Thanks. Yes there are several condition that can be checked by pre-implantation genetic disgnosis and also that can be done by amniocentesis or Chorionic Villus Sampling, to see if the baby is affected. I suggest that you speak to a geneticist, who can do a test run on possible obvious inheritance profile, and advise accordingly. After this, then speak to your Fetal Medicine Specialist Obstetrician

A. Thanks. I have endometriosis but I think something else may be wrong, please help!

I have had no periods for the last 6 years I have had 4 pregnancys 2 full terms 1 miscarriage 1 abortion I have had blood clots ( no blood) and 3 bleeds in 6 years I have a swollen stomach at random times pain and pressure in my left had side I am in pain during sex, I have had the same partner for over 6 years and constant tiredness and lack of energy and I have black outs where I completely forget things I have had a laparoscopy that showed endometriosis but I still feel there’s something else wrong! i could really do with some advice, have you any idea what this could be? Please help ,
 
A. Thanks. You do sound like you are likely to have endometriosis. The pre-menstrual and peri-menstrual symptoms associated with this can be quite disabling sometimes. You do need treatment, which may involve hormonal and sometimes drastic medication, such as Zoladex. I hope this helps you. Take care.

I had a c-section and still suffering from heavy bleeding, what should I do?

Q. Hi, I just wondered if you could help me with something? I Had a c section 5 months ago and was given the pill injection after two weeks after the c section. I’ve been bleeding for 5 months, which only stopped for 3 days in this 5 month period. I then had 2 pill injection from the doctor every 9th week so far to stop the bleeding and it has not worked. I was then given tablets which slowed it down but didn’t stop it. I’m very worried now, I don’t know what’s causing this. Can you help?,
 
A. Thanks for your question. I think there is some hormonal imbalances or more appropriately hormonal disturbance here. It may stem from how your body tries to adjust to the hormonal changes of after pregnancy, while at the same time, coping with the new hormonal injection. I suggest you speak to your GP, to refer you to a gynaecoloigst. They may wish to do further tests. Take care.

I have not recovered vaginal tone since delivering my baby. Is vaginal tightening surgery an option?

Q. Hello Dr Daniels and thank you for your time. I gave birth 2 years ago to a beautiful 9 pounds baby. I have not recovered by vaginal tone yet, since air comes in an out because the vagina walls are not closed.
Is the vaginal tightening surgery useful in this case? Thank you for this!,
 
A. Thanks for the question. I think that it is going to be very difficult to get more “tone” to stop the vaginal wind – it is usually due to damage to the transverse perineal muscles – which form the “vagina gate” against air-trapping – A vaginal tigthening will help that, but the tigthening must include, not just vaginal narrowing, but also closure of the transverse pernieal muscles – as in a properly done perineoplasty – with this in mind. What could cause recurrent miscarriages?

Q. If all blood clotting types are ruled out also cromesonal abnormalities what could possibly cause recurrent miscarriage ? (I have a healthy son and have had 4 losses at 5 weeks pregnant over 2 years),
 
A. Thanks. The two are very broad causes, and its difficult to be specific, as to your cases, as age, medical background, uterine normality all need to be exlcuded. I am assuming here that you have been tested for all Leanticoagulants, and antiphospholipid antibodiies, etc. and that you have had histology of the fetus, your own chromosomes have been analysed and that you have taken folic acid for three nonths before conception and that you doctor have tried you on emperical low dose aspirin …???? these are some things you will need to consider if you haven’t already. If I can be of further help, perhaps as to some of things I have mentioned here, please don’t hesitate to contact me again.

I am 35 weeks pregnant and think I have a vaginal prolapse?

Q. hi im 35 weeks pregnant and i think i may vaginal prolapse , i was using a mirror whilst shaving as i cant see down below at the moment and went to check that everything is all good and noticed that the entrance of my vagina is very bumpy and i have this one bumpy lump that sticks out from the inside it doesnt hurt but it quite large , for the past two years i have suffered with more discharge and sperm takes ages to come back out so i have to ware pads all the time as to me the lump acts like a block , i am suffering with spd at the moment and i have had a child 4 years ago , what are your opinions on this thanks,
 
A. Thanks. There is really no options during pregnancy, apart from putting a vaginal ring or other types of vaginal pessaries, if it’s troublesome – this is usually unnecessary. Most of the time – it will get better after three months from delivery. So it’s ok to review the findings then, with your GP

A. Thanks. You do not need any hormonal treatment. As you are 37 you need to be aware of increased risk of abnormal formation, so, do use folic acid three months beore trying again, and go to doctor as soon as you are pregnant
