Q. my girlfriend experiences slight tearing in the 6 o’clock position of her vagina during sex. the tear isn’t apparent during, but she feels it shortly after and takes several days to heal. she describes it as a paper cut like sensation, or a toothache like pain. it happens every time, she is well lubricated and we use extra lubrication, we are gentle and don’t prolonged sex for fear of hurting her. she had a baby 10 years ago and tore during delivery, she hadn’t had a sexual relationship since then until we met six months ago. she is 39 years old and didn’t have this problem in relationships prior to giving birth. what can be done to enable us to have a normal sex life?,
A. She probably need to have the vaginal introitus enlarged, by removing the offending band of skin – it can be done under loal anaesthesia. If you get stuck, do made enquiry via info@gynecosmetics.com

Q. my wife has an epistiotomy that opened up after birth, hospital said its one of the most severe cses they have seen, she is getting lots of pain in over granulated tissue and a discharge that has not stopped for 3 months. No one is helping, they want to restitch or cut the tissue but not yet and wait another month, but no one is saying what the discharge is or why the pain is in all surrounding areas? pls help! i have found a private surgeon who wants to operate now but the nhs doc wants to wait 4 – 8 weeks more. ,
A. The decision to operate is always taken with courage. I have done this before, with a lot of counselling of the patient, but every patient is different. It may take three months or more to get ready if one is to wait before stitching

Q. Is it ok to exercise with two recently diagnosed ovarian cysts sized 3.7u and 5 cm?,
A. Should be OK, but depending on the type of cysts, the 5cm one may be prone to twisting, and that amy cause severe pain, and you may need to be seen in the hospital for this. It really depends on the type of cysts you have. It is unlikely that the 3.7cm one wil twist.

Q. Hi
I am really sorry and I dont really know where to start, I am five weeks pregnant with twins. I have had twins before and both me and my children suffered from life threatening complications. I was refered to my local unplanned pregnancy unit today who told me I should have a termination as I was not well enough to carry two babies. When I asked if there was the possiblity of reducing thie pregnancy to just one baby, then the risks would be reduced her answer was ‘this is not something that I deal with and is wrong and cant be carried out before 16 weeks and how would you choose and went on to say its fairer to terminate both’. I have understandably come home very upset and have looked into the possibilities of a reduction and have found that this can be carried out between 9 and 12 weeks, and that the selection as it is my health in question and not the babies would purely be done on the safest one to get to etc.
I know that this ia maybe not an area that you deal with or maybe even agree with but I do not want to loose both of my babies if I can help it. Im trying to find some independant advice and I was wondering if you knew or could recommend anyone who might be able to help. I do not mind how far I need to travel, I live in salisbury. And this is not something that I have taken lightly.
I look forward to hearing from you with the hope that you might beable to point me in the right direction.
,
A. Thanks. Professionally, I do not make judgement on the rights or wrongs of this matter. However, It is best in my opinion for you to have a candid discussion with your GP who will them either referral you to a local hospital or to a fetal medicine unit – a little further from Salisbury, where your case will be individually considered and handled accordingly.

Q. Mr Daniels,
My daughter gave birth 10 weeks ago to a healthy baby girl. There were complications at the birth insofar as the baby was in distress. In addition to the pressence of the Midwife Consultants were present at the birth and I am advised it was a Consultant who delivered the baby. My daughter in the last 48 hours was admitted to A&E at her local Hospital where she has been diagnosed with a Uterine Prolapse with the suggestion being it may have been caused during the childbirth by the use of forceps damaging the uterus. My daughter is now facing surgery, hospitalisation and inability to conduct a normal life for 6 weeks but most upsetting of all the realisation that she will be unable to have a natural birth with any subsequent child. Overcoming the difficulties ids the primary concern however can I seek your opinion as to whether the outline details that I have provided can b attributable to professional error at the time of the birth. ,
A. This is a legal question of professional liability or negligence. I cannot make comments without looking at the evidence. Suffice to say that it can happen, after normal delivery without the use of any instrument. It seems to be a little mor than just “uterine prolapse” though if she is so incapacitated..

Q. Hello,
I had both my front and rear walls repaired July 12th this year and have found that I have a piece of skin about the thickness of my middle finger running across the centre of my vagina. I cannot have intercourse with my husband as when he tries he bumps this piece of skin and the pain brings tears to my eyes.
It’s really strange as I can insert my finger and can push my finger past this piece of skin on both sides and can feel behind it, that’s how I know it’s rough thickness.
The best way to describe it is if you imagine a toilet roll as my vagina and then push a pen through from one side and out through the other and then look through the toilet roll with the pen running top to bottom, that’s how I feel inside. Like a no entry sign on roads with the line running vertical.
Please can you help?
I have a follow up appointment with my gynaecologist the 29th September (this month).
Kind regards
,
A. I think you might have a scar tissue running across, this may need to be divided or freed up. Its not a major concern usually – at least from the gynecoligist viewpoint. It does happen, but now rare. Sometimes it helps to examine patients at the initial phase after this surgery, but one would have to do several of those examinations to prevent one case like this one.

Q. I have had a throbbing pain on the left hand side of my vagina for the past week. I was treated for thrush as I had itching and burning. This has now cleared up but I am still suffering discomfort from the throbbing. I have felt on the outside for a lump and think I can feel a small one on the inside when I put pressure on it. Should I be worried about it or will it clear? Any advice would be greatly appreciated. Thank you,
A. IF you do feel a lump, do have your GP or gynaecolgist check this out.
